Types of health insurance plans

Health Maintenance Organizations (HMOs): HMOs require you to choose a primary care doctor who will coordinate your care.

Preferred Provider Organizations (PPOs): PPOs allow you to see any doctor or hospital that you choose.

Exclusive Provider Organizations (EPOs): EPOs are similar to HMOs, but they do not allow you to see out-of-network doctors or hospitals at all.

Point-of-Service (POS) plans: POS plans combine features of HMOs and PPOs.

High-Deductible Health Plans (HDHPs): HDHPs have lower monthly premiums than other types of plans, but they also have higher deductibles.

Catastrophic plans: Catastrophic plans are designed to cover only major medical expenses.
